Signs of Suicide (SOS) - New Horizon Mental Health Services

Parent/Guardian(s),

If your child is taking Health 1st semester @ PHSN, they will be participating in a suicide prevention lesson instructed by counselors from New Horizons Mental Health Services. This lesson is called the Signs of Suicide (SOS) program and it will take place throughout September at PHSN during their Health class. Attached you will find a letter that gives you more detail about the lesson as well as an area to sign if you do not want your child to participate in the screening. This letter will also be sent home with each student in the upcoming weeks.

Thanks,
PHSN Health Dept.
