Reminder For Student OneView EMA Form

If you haven’t processed your student’s OneView Emergency Medical Authorization (EMA) Form, we’re asking all custodial PLSD parents to do so as soon as possible.  We strongly encourage parents to complete the EMA form in advance of the school year, in order to make the back to school process as seamless as possible for parents and students.

Below are a few tips/reminders in regard to the OneView process:

  • Only one custodial parent can attach a student and process the EMA Form.
  • Access for OneView is now through Infinite Campus Parent. It is essential that custodial parents have their Campus Parent account set up and know their login information prior to the Welcome Days at each building. Additional information regarding Campus Parent can be found on the district website.  If you need additional assistance with Infinite Campus, please submit an Infinite Campus Help Desk ticket.  After logging into Infinite Campus Parent, the link to OneView is on the opening Message Center “Announcements” screen.
  • You will need the student ID number if attaching a new student. The student ID number is the student lunch code.  After logging into Campus Parent, select “Today” in the options under the three line menu icon, then select your student. The student ID number will be listed below the student name.
  • Junior high and high school students will not receive their schedules until completion of the OneView form has been confirmed.
  • Refer to the District’s Back to School webpage to find step-by-step directions for OneView.
  • The “Quick Submit” button is new this year and is for returning students who submitted a OneView EMA form last school year. Although the Quick Submit button is designed to speed up the process of filling out forms, parents are strongly encouraged to verify the accuracy of all info and review all agreement/consent items thoroughly before clicking this button.
  • Visit the OneView “Frequent Questions” page as a troubleshooting resource.
  • Submit a OneView help desk ticket if additional assistance is needed.
