RELEASE OF INFORMATION
If your physician is requesting teachers to complete assessments/scales for your child, you will need to complete a Release of Information form. After completing the release, return it to your child's school counselor. Once the release is received, teachers will then be permitted to do the assessment/scale. The release form is located on the "Counseling Information & Links" tab or in the guidance office.
